Rules to participate: I do represent and certify that I am the parent or legal guardian of the minor child (of less than 21 years of age) named above and that I have the legal right to request and sign the liability release for this child. I hereby release, discharge and agree to save harmless the News & Record, the ACC, the Greensboro Coliseum, their legal representatives or assignees, and all persons acting of any nature which may arise out of my child's attendance of the ACC Hoops for Kids to be held at the Greensboro Coliseum, Thursday, March 4th, 2010. I give permission to the ACC hoops for Kids authorities to have my child transported and to a hospital and treated by a physician. For Groups of 10 or more please send all registration forms together.
